There are some citations that OSHA inspectors write over and over again; they’re in the top 10 every year. And yet they’re some of the easiest citations to avoid—particularly because employers should see them coming in time to get out of the crosshairs.
Lockout/tagout citations tend to be classified as “serious” because failing to control hazardous energy while workers are cleaning, repairing, servicing, setting up, or adjusting prime movers, machinery, and equipment can result in catastrophic injuries, amputations, and deaths. For each piece of equipment, you must have a written step-by-step energy control procedure, and workers must receive training appropriate to their job duties. The procedures must be evaluated annually.

David W. Smith, CSP, P.E., is the principal of Ensign Safety in Clovis, California. He has 45 years’ safety engineering experience and over 30 years of industrial hygiene experience with an emphasis on lockout/tagout, air sampling, noise, and other exposures.
Smith studied chemical engineering at Brigham Young University and has regulatory, compliance, and industry experience with significant success in reducing industrial accidents and safety-related costs. He has a combined 27 years of field experience as the safety director for an agricultural products processor and as a senior safety engineer for two insurance companies. Appointed by California’s governor in 1984, he served seven years as a board member of the California Occupational Safety and Health Standards Board (the state agency developing all occupational safety and health regulations) and continues to serve as an expert witness in safety standards and accident evaluation in over 200 cases.
As the owner of Ensign Safety, Smith has provided professional services to clients in California and internationally for over 20 years, including consulting and lockout/tagout services. In 2005, he developed quality software designed to write machine-specific lockout/tagout procedures.
