Required by both the Department of Transportation (DOT) and the Environmental Protection Agency (EPA), the “Uniform Hazardous Waste Manifest” is the shipping document that travels with hazardous waste from the point of generation, through transportation, to the final treatment, storage, and disposal facility (TSDF). This one-page form is at the heart of the EPA hazardous waste management system. It tracks the chain of custody for the waste from its point of generation until its final disposition. Each party in the chain of shipping, including the generator, signs and keeps one copy of the manifest, creating a “cradle-to-grave” tracking of the hazardous waste.
